SCHOLARSHIP APPLICATION

Every qualifying application will be considered for the CGSF Phonothon Funded Scholarships. However, Donor Sponsored Scholarships are also available ($1,000 and up) to qualified students. These funds have been established in honor of or in memory of a person with ties to the Center Grove community. Often the source of funding is from the family itself although pledges through the annual CGSF phonathon or other fund raising events contribute as well.

PLEASE READ: The questions below will help us determine which scholarships you may qualify for. Please answer these questions and then after carefully reading the following criteria, check the box of the scholarships you feel you qualify for. There is not a limit to the number of Donor Sponsored Scholarships you may choose.

DONOR SPONSORED SCHOLARSHIP

	Amy Dillman Black Memorial Scholarship - This scholarship is in memory of Amy Dillman Black, a 1999 CGHS graduate who was an outstanding student, Class President, and Debtone. The scholarship is awarded to a student who is pursuing a degree in nursing.
	Center Grove Alumni and Friends Scholarship - awarded to a Center Grove senior who has a family member (excluding siblings) that graduated from Center Grove.
	CG Band - based on need for a 4-year member of the band program. Include essay entitled "Far reaching effects of life lessons developed through band"
	Christian "Binky" Nikou Memorial Scholarship Fund - This scholarship fund has been established in memory of Christian "Binky" Nikou, a Center Grove High School alumnus from the Class of 2006. While at Center Grove, Binky was a member of an All-State Doubles Team in 2002, and was an All-State Singles performer in 2004, and 2005, when he was the Singles State Champion. Selection criteria for this scholarship is still being finalized.
	Class of 1965, Margaret and Vandy Scholarship - This scholarship is given by Vandy’s Kids and the CGHS class of 1965 in memory of Melvin “Vandy” Vandermeer, a former beloved prgincipal at CGHS and Margaret, a foreign languages teacher at CGHS. Along with the completed application, candidates will provide two letters of recommendation from teachers at CGHS* and plan to major in engineering, physics or mathematics at an accredited university. Additionally, successful candidates will demonstrate academic excellence, significant participation in non-academic activities, and outstanding moral character. It is funded by private donations from friends and family. *Note: Applicants for this scholarship must provide two typed recommendations from two CGHS teachers. Please bring these to the Guidance Office by the application deadline. Make sure your name is on each one.
	David A. Hicks Memorial Scholarship / CGEF - This award honors David Hicks, well known for his involvement in many youth activities, especially Junior Achievement of Johnson County. Criteria include exemplary community service and school activities involvement. It is funded through contributions made through the phonothon.
	Denney G. French History Scholarship - history; history teaching; strong academics; leadership; school/community involvement.
	E. Lee Webb/CG Optimist Club - strong academics; leadership qualities; school activities.
	Friends Forever Memorial Scholarship - This scholarship honors the memory of three CGHS graduates, Joseph Alexander, Nicholas Habicht, and Jacob Surface, who died from carbon monoxide poisoning in a house fire while attending IU in May 2004. It is awarded to a senior attending IU-Bloomington, possesses a positive attitude, has drive and determination for a successful future , and is involved in extra-curriculars. It's funded by a Friends Forever Memorial August golf outing. Nic's sister, Malia Casper, teaches first grade at CGES.
	Jarrod Tyler Polston Memorial Scholarship - This scholarship is awarded to a CGHS student who played 4 years of football at CG, has a 3.0 GPA, and has been accepted to an accredited college with preference for IU-Bloomington. Jarrod, a kind hearted young man who loved his friends and family, was pursuing Biochemistry/pre med. One of his proudest moments was on the field at Lucas Oil Stadium when CG won the 2008 State Championship. He loved football, his teammates, and football family. Funding is provided by contributions and the family.
	Jason Mueller Memorial Scholarship - This scholarship fund has been established in memory of Jason Mueller, who passed away July 22, 2021. Jason graduated from Center Grove High School in 1994, playing both football and baseball for the Trojans. For the past 23 years, he worked for the Center Grove Community School Corporation, teaching fifth grade for 13 years and elementary physical education for 3 years. He is most known for that recognizable voice: serving as the public address announcer for a variety of sports and events. He IS the Voice of the Trojans. Preference is given to a student majoring in education.
	Jean Allen Robertson Memorial Scholarship - This $2,000 scholarship honors Jean Robertson, dedicated CGHS English/Social Studies teacher (1956-1975), WRT resident & parent. It is awarded to a student studying education, social studies or music at an Indiana public university. Academics, financial need, & motivation are also criteria. It is also in memory of the Gregg family, original owners of the family's WRT family farm.
	Judith A. Meeks Choral Music Scholarship - in honor and memory of Judith A. Meeks, founder and long time director of CGHS's award winning choral groups. Criteria include the CGSF standards, membership in CGHS performing choirs, and the intention of studying vocal performance, vocal music education, or musical theatre in college. Funded by the CGHS Choir Parents Organization and private donations.
	Julie Johnson Memorial Scholarship Fund - The scholarship is in memory of Julie (Eck) Johnson, who provided years of speech-language therapy services to students across the Center Grove School District. She was dedicated to the goal of helping her students communicate and have a voice. She impacted many lives with her smile and compassion. Julie's determination to overcome health obstacles and her commitment to helping students overcome communication and learning obstacles was inspiring. This scholarship is intended to extend Julie's fight to help others overcome barriers and challenges. It will be awarded to a student who has shown determination to overcome challenges.
	Kenzie Holubar Memorial Scholarship - This privately funded scholarship is in memory of Kenzie Susanne Holubar, infant daughter of Bart and Angie Holubar, sister to Avery, Ajay, Brody, and Brevin. She was born and passed away on June 16, 2015. It is awarded to a senior with a minimum 3.6 GPA who participated in school activities and will attend college in Indiana or Michigan.
	Leslie Cook-Dickerson - strong academics; leadership qualities; school activities involvement, 4 year college and pursuing a health related career.
	Pamela A. Todd Memorial Scholarship - This $1,000 scholarship is awarded to a CGHS student who meets the CGSF standards and will be attending an accredited college. Applicants should be active in CGHS athletics and other activities. Funding is provided by private contributions, especially from Texas Roadhouse.
	Rachael Hope Valentine - strong academics; leadership qualities; school activities involvement, 4 year accredited college.
	Russ Milligan Memorial Scholarship - Russ taught Chemistry at CGHS for 31 years and was the Varsity Girls Softball Coach for 27 years. He will forever be remembered for his live of teaching and love of high school athletics.
	Sandra Hillman Early College Scholarship - This scholarship is in honor of long time CGHS Assistant Principal Sandra Hillman, also an advocate of the Early College Program. Applicants must meet all of the CGSF criteria as well as having graduated from the Early College Program. It is funded by CGHS staff and parents as well as the community at large.
	Sheila Crump Memorial Scholarship - This $1000 scholarship is awarded in honor and memory of Sheila Hunter Crump whose 25 year career at CG was dedicated to improving the lives of students with disabilities. Sheila, a 1981 CG graduate, touched the hearts and lives of students, families and colleagues. Preference will be given to students who have been involved with Peer Tutoring, Best Buddies, Unified Sports, or have received services and plan on attending an accredited vocational school or college. It is funded by donations from family, friends, and fund raising efforts, as well as the CGSF phonothon.
	Steve McClure Memorial Scholarship - This scholarship fund has been established in memory of Steve McClure. Steve was an announcer with The Center Grove Sports Network.
	The Dale and Carol Tumey Scholarship - Dale and Carol Tumey Scholarship-female athlete, 4 year accredited college. Preference to secondary education majors and those attending Franklin College. The Tumeys reserve right to select recipient.
	The Danner Family Perfect Attendance Scholarship - This $1,500 scholarship is awarded to a graduating CGHS senior who has attained perfect attendance during grades 1-12 with at least 10 years in the Center Grove Community Schools (including no days tardy and no early dismissals). The student must meet CGSF standards and be planning to attend a North Central accredited Indiana college or university. In case of a tie, GPA will be considered.
	The Flynn Family Music Scholarship - This $1,000 scholarship is awarded to a Center Grove senior planning on pursuing any music major in college. Criteria include two years minimum participation in any music activity or group or theatre activity while at CGHS, acceptance into an accredited college or university, as well as fulfilling the remaining CGSF basic requirements. It is funded by the Flynn Family.
	The Joyce Hall Memorial Scholarship for Aspiring Teachers - This $1,000 scholarship is awarded annually to a student who is planning on entering the field of education. Joyce Hall was a long time member of the CGCSC school board at a time of explosive growth in the district. Many of the programs and buildings one sees today are a result of Joyce's activism. Joyce was devoted to the school community and the success of all students. Criteria include meeting all of the CGSF requirements including acceptance into an accredited college. The scholarship is funded through donations from family and friends.
	The Newport Family Robotics Scholarship - This $1,000 minimum scholarship is awarded to a senior who has been active in the Robotics program at CGHS, especially during senior year. Students must also meet the CGSF criteria. It is privately funded by the Newport family.
	Zachary Scott Schafer Memorial Scholarship - Zach, Class of 1989, was a swimmer, diver, wrestler, and champion pole vaulter at CGHS. This scholarship will be awarded to a boy with average grades who exhibits the same qualities that he had.

Please submit a short essay/statement (250-500 words) that tells us something about yourself. Possibilities include your personal and academic goals, where you see yourself in ten years, what activities in high school or in the community have helped shape or guide you. You may wish to talk about the influence of a teacher, family member, etc. Include any special financial or family circumstances that would be helpful in getting to learn about you.

Please note: This statement may greatly influence the decision of Donor Sponsored Scholarships so please be thoughtful and thorough in your statement if you wish to be considered.
