SCHOLARSHIP APPLICATION

Every qualifying application will be considered for the CGSF Phonothon Funded Scholarships. However, Donor Sponsored Scholarships are also available ($1,000 and up) to qualified students. These funds have been established in honor of or in memory of a person with ties to the Center Grove community. Often the source of funding is from the family itself although pledges through the annual CGSF phonathon or other fund raising events contribute as well.

PLEASE READ: The questions below will help us determine which scholarships you may qualify for. Please answer these questions and then after carefully reading the following criteria, check the box of the scholarships you feel you qualify for.  There is not a limit to the number of Donor Sponsored Scholarships you may choose.

Scholarships highlighted in green below, have been determined relevant based on how you answer the questions below.
Are you pursuing a degree in nursing?

Do you have a family member (excluding siblings) who graduated from Center Grove?

Were you in the CGHS band?

Did you play Tennis at Center Grove?

Do you plan to major in engineering, physics or mathematics at an accredited university?

Have you been involved in an exceptionally large amount of community service projects?

Do you plan to study history or political science?

Have you participated in a significant amount of community service activities?

Are you planning on attending Indiana University-Bloomington?

Did you play football at CGHS for 4 years?

Are you pursuing Education, Social Studies or Music at an Indiana public college?

Do you plan to study musical voice performance or voice music education?

Are you attending college in either Indiana or Michigan and have a GPA of at least 3.6?

Do you plan on pursuing a medical career?

Were you on the CGHS swim team?

Are you in Early College at CGHS?

Have you been involved with Peer Tutoring, Best Buddies, Unified Sports, or received services and plan on attending an accredited vocational school or college?

Were you a female athlete at CGHS?

Do you have PERFECT ATTENDANCE during any 10 years at Center Grove Community Schools?

Have you participated in any music or theatre group or production for at least two years and do you plan to study music in college?

Do you plan on studying education in college to become a teacher in K-12?

Are you involved in the CGHS Robotics program?

PERSONAL STATEMENT

Please submit a short essay/statement (250-500 words) that tells us something about yourself. Possibilities include your personal and academic goals, where you see yourself in ten years, what activities in high school or in the community have helped shape or guide you. You may wish to talk about the influence of a teacher, family member, etc. Include any special financial or family circumstances that would be helpful in getting to learn about you.

Please note: This statement may greatly influence the decision of Donor Sponsored Scholarships so please be thoughtful and thorough in your statement if you wish to be considered.

ACCEPTANCE OF TERMS

By submitting this application, you are agreeing to notify CGSF should you become ineligible for a general scholarship by receiving scholarship aid of over $5,000. You are agreeing that your official transcript will be reviewed and verified. You agree that if you are offered and accept a scholarship from CGSF, CGSF may use your name, the name of your community, the name and address of your school, the amount of the award, and the name of the post-secondary institution you will attend in press releases, public announcements, and other fundraising or promotional materials in all media, to advance the non-profit objectives of CGSF and its affiliated programs.
